How about the specifications
of the photo for the NHI IC Card? Can I use a photo without
the requirements? |
 |
This photo should be 2-inch,
colored/black-and-white and taken within 2 months. On
the photo, the figure shall be without a hat and shall
not wear the colored glasses. We don't accept any photo
other than a certificate photo. If the specifications
of your photo are almost close to the above-mentioned
but only the size is 1 inch, we could accept it. However,
the clarity of the photo on the NHI IC Card will not be
good as the 2-inch one. |

What are the contents
in NHI IC Card? |
 |
There are four sections of information
stored in NHI IC Card, including the personal information,
the NHI-related information, the medical services and
the public health administration. The contents are as
follows,
- The Personal Information Section mainly includes the
card's serial number, the cardholder's name, gender, date
of birth,ID number, picture and the date of issue.
- NHI-related Information Section mainly includes the remark
of the cardholder's status, the remarks for catastrophic
diseases, the number of visits and admissions, the utilization
of the NHI health prevention programs, the cardholder's
premium records, the records for accumulated medical expenditures,
the amounts of cost sharing, etc.
- Medical Services Section mainly includes the drug allergic
history and the long-term prescriptions of the ambulatory
care and certain medical treatments. This section will
be gradually phased in depending on how the healthcare
providers adapt themselves to the system.
- Public Health Administration Section mainly includes
the personal immunization chart and the willingness for
organ donation.

How to have another
NHI IC Card if the original one had lost or damaged? Should
I pay a handling fee? |

If your card is lost, stolen
or damaged, please contact the BNHI for replacement. Please
mail your NHI IC Card application form along with the
photocopy of your ID to the Bureau of National Health
Insurance. You may also hand in to the BNHI branch. A
handling fee of NT$200 will apply. |

When can I start using
the NHI IC Card? |
 |
You can start using the NHI
IC Card from July 1, 2003; however, the current NHI
paper card is still valid. A photo ID will be needed
to identify
yourself if you hold a non-photo NHI IC Card. Moreover,
additional copayment for high frequent outpatient visitors
will not be charged for the year 2003, only registration
fee and the usual copayment will be collected for all
patients. |

How do I update the
NHI IC Card If I have used it for six times already? |
 |
You can update your card at
the Public IC Card Kiosks installed at all BNHI branch
offices and affiliated outpatient centers; or look for
the IC Card Readers installed at nearest hospitals, or
local town houses. |

What to do if I have
not received the NHI IC Card? |
 |
You can still use the paper
card. Your NHI IC Card is delivered to your group-insured
unit first and may take some time to reach you. But you
may call 0800-030-598 to ask for the status of your NHI
IC Card. The existing NHI card renewal spots provided
by BNHI will continue to provide card renewal services. |

What should I do if
the NHI IC Card shows "validity expired" when I use it? |
 |
The validity of the NHI IC Card
should be re-approved once a year. Its validity period
ends on your birthday of the next year, so please renew
your card within one month prior to your date of birth
by using the card readers every year. You may also ask
the hospital staff to help you renew it. |

Is it necessary to bring
the NHI IC Card with me when seeking Chinese injury care,
acupuncture treatment, or dental root canal? |
 |
Yes, the NHI IC Card will be
swiped even for treatments of the same episode of diagnosis,
but it does not count for one visit time. |

Can I set a password
for the NHI IC Card? And how do I set my personal emergency
phone number? |
 |
Since there is no medical history
built inside the IC card, we do not suggest you to set
a password for now. However, in times of need, you may
use the card readers installed in all BNHI offices and
local town houses to set your password and emergency numbers. |

Is there any possibility
that my medical information will be revealed without permission
when using the NHI IC Card? |
 |
The NHI IC Card is to be read
by the NHI IC Card readers only, and each reader needs
a BNHI SAM card to function. Data stored in the cards
cannot be read without going through a strict authorization
and mutual recognition security process. Besides, there
will not be any medical history recorded in the NHI IC
Card so far. |
